The flashing green light on a smartwatch estimates a person's heart rate by measuring the amount of light reflected back to the watch as blood vessels expand and contract.

Findings
Additional insights we found via Royal Society of Chemistry
Heart rate measurement, or photoplethysmography, relies on green light because blood absorbs it.
Each heartbeat dilates vessels, allowing them to absorb more green light and reflect less, while contracted vessels do the opposite.
The data on reflected light intensity are converted to heart rate using an algorithm, though inaccuracies arise due to variations in skin pigmentation.
Some smartwatches also measure blood oxygen saturation by measuring how much light of certain wavelengths is absorbed.
