Women have typically been underrepresented in clinical trials, and this podcast says about 30% of new drugs aren't adequately tested on women. Differences in how men and women respond to medications are due to factors such as hormonal differences, fat distribution, water content, immune system responses, and more.
There's been particular reluctance to enroll women in clinical trials since the thalidomide tragedies in the 1950s, when a large number of women using thalidomide to address nausea during their pregnancies had children with severe birth defects related to the medication. The subsequent concern has been that women in a trial may become pregnant, and their exposures could harm fetuses.
Some research finds that immune responses to the flu vaccine differ by sex, with women showing a stronger response to the shot but also more side effects. This result supports the idea of considering sex-specific medication dosing.
