  1. After the disintegration of the space shuttle upon re-entry due to falling debris that damaged Columbia's wing during takeoff, rocket design shifted to placing the crew atop the rocket, rather than to the side.

  2. At NASA, safety culture underwent numerous changes, including replacing audio conferencing systems with video systems, replacing rectangular tables in meeting rooms with round tables to encourage participants to speak up, and setting aside time for engineers to consider organizational improvements to safety.

  3. Before flights resumed following the disaster, NASA mandated that a spare shuttle always be ready to rescue crew members in orbit if their shuttle was damaged during launch.

  4. After the fleet's retirement in 2011 but before commercial partners, astronauts remained spacebound via Russia's Soyuz vehicle, which had been transporting cosmonauts to the International Space Station.
