Beyond technical failures, the Challenger disaster exposed deep flaws in NASA's leadership.

Findings

Additional insights we found via The New York Times

  1. Known issues with the shuttle's O-rings, whose failure caused the disaster, were deemed acceptable risks, in part to ensure President Reagan could cite the launch's success in his State of the Union address.

  2. Delaying the launch would also have undercut the publicity surrounding educator Christa McAuliffe's planned live lessons from space.

  3. The subsequent investigation found that NASA had normalized known risks and that communication between engineers and leadership had broken down.

  4. The disaster grounded the shuttle fleet for nearly three years and prompted significant changes to NASA's safety oversight and decision-making culture.
