After Bernie Madoff's nearly $65B Ponzi scheme was exposed, the Securities and Exchange Commission was widely criticized for being inattentive and overly lax for not having caught the fraud earlier.

Whistleblowers from Madoff’s investment advising firm had written to the SEC as early as 1999 to say that Madoff’s money management was illegal.
A follow-up investigation into the SEC’s handling of the case found that the agency had substantive enough information to issue a warrant and complete an investigation well before Madoff admitted to running the scheme.
A Ponzi scheme runs on investments from new clients to pay out previous clients, without any additional outside capital being added to the fund through profits.
